When completing the intake form:

This information is necessary to tailor the massage to your health and preferences, ensure your safety, and to be able to contact you if needed.

By completing the intake form, you explicitly consent to the processing of your (sensitive) personal data for the purpose of massage treatment.

4. How long do we keep your data?

We store your data:

For up to 2 years after your last appointment
Or longer if required by law (e.g., for administrative or tax purposes)

We do not share your data with third parties, unless:

You have given explicit consent
We are legally obligated to do so

You can send a request for access, correction, deletion, data transfer, or withdrawal of your consent to helderinharmonie@gmail.com. To verify your identity, please attach a copy of your ID. In this copy, please black out your photo, MRZ (machine readable zone, the strip with numbers at the bottom of the passport), passport number, and Citizen Service Number (BSN) for your privacy. We will respond to your request as soon as possible, but no later than four weeks.
